At the time of consultation you will be given a statement for your proposed procedure(s) including the date and total cost. The total cost will include the Surgical Fee, Facility/Anesthesia, Materials/Implants and Injectables. A Scheduling Deposit is required, applied to the total cost and is refundable prior to 21 days before surgery. The Balance of Payment is due 21 days prior to surgery. There will be a preoperative visit scheduled in the week or two prior to surgery for further discussion, examination, preoperative photography and medication prescriptions.

Included in the total cost are the preoperative visit, preoperative and postoperative photography, immediate post-surgical garments, and postoperative care for one year.

Not included in the total cost are preoperative fees to laboratories or clinicians for your presurgical testing and medical clearance, prescriptions for pain, antibiotic and antinausea medications, private duty nursing (if you choose, in which case you would pay directly to the nurse), postoperative fees to pathology or microbiology specimen analysis, additional postoperative garments, hospital/anesthesia expenses related to intraoperative/postoperative complications which may or may not be covered by insurance, and office facility and anesthesia fees for revisionary surgery.

Fees are valid for 90 days from date of quotation. We accept cash, bank check, wire transfer, and credit cards (Visa, MC, Amex).

Cancellation policy:
– 15 to 21 days prior to surgery full refund minus scheduling deposit
– 8 to 14 days prior to surgery 50% of total cost
– 0 to 7 days prior to surgery no refund
Cancellation for any reason other than physician-documented medical necessity will be subject to the cancellation schedule above. Rescheduling of surgery for physician-documented medical necessity will incur a reschedule fee.
